如欲分享學與教資源到教城，請填妥本表格，然後按「遞交」。香港教育城將於7-10個工作天內聯絡閣下。如有任何查詢，請致電2624 1000或電郵 info@edcity.hk。
表格一經遞交，即表示閣下同意遵守教城各項服務條款及私隱政策，並受該等服務條款及政策約束。
